Catalytic Health Partners


Catalytic Health Partners (CHP) is a population health management company with a mission to reshape healthcare. CHP contracts directly with health plans to manage Medicare Advantage, Medicaid and dual eligible (Medicare and Medicaid eligible) individuals, we call members, to provide integrated care. CHP has an unwavering focus on accountability and great member outcomes by addressing the entire Catalytic Health Triangle: social, behavioral and physical medicine.

Our member care model is built on the foundation of people, technology, and data. We combine caring, personalized, in-home member engagement for those most at risk for poor health outcomes with convenient digital connectivity and advanced current analytics. Our proprietary technology includes our health app, CHamP, and CHPCareCentral, a web-based population health management software and our centralized database with an integration of clinical, claims and member generated data on a longitudinal basis.

CHP turns data into information, information into insight, and insight into timely, exceptional and compassionate member care. Since 2014, we have managed more than 1,000 members and more than 80% of all CHP members have had improvements in their health outcomes with significant cost-savings.

In addition to a full-service care team of health professionals, each member is provided an internet connected tablet with access to the proprietary CHamp health app, which includes telemedicine, telemonitoring, medication adherence reminders, and a portal for educational videos and access to print materials.

We first address their social needs, tapping community resources or funding them ourselves. Our data indicates 65-70% for our population’s healthcare costs are related to social needs. Our care coordinators and social workers work to ensure that these needs are met in a timely manner. CHP behavioral health counselors and psychiatric nurse practitioners help to support change management over and above behavioral diagnosis management. The medical team works to ensure physical health needs are being met and closely monitors telemonitoring data to support their work.

We utilize our CHamP health app to support our engagement and care for our members for important conversations with them, monitoring their vital statistics, as well as providing educational materials.

We have identified some opportunities to improve the functionality of the app to better support our members in their success. Our ability to scale is critical in a mission to improve, change and save lives. It is our goal to transform more than 1,000,000 lives over the next 10 years by being the standard for healthcare delivery demonstrating excellent outcomes with lower costs. Investing in our proprietary health app, CHamP, and CHPCareCentral, our web-based, platform population health management software is fundamental in achieving this goal.

What problem does this idea/product solve or what market need does it serve?

Healthcare costs continue to escalate with declining health outcomes; health plans are finding it difficult to reduce costs due to a lack of integrated healthcare. The market, patients, providers all agree there needs to be a solution beyond government mandated cost-containment. It requires a new approach to managing high-risk, high-cost and low engagement members. At Catalytic Health Partners we’ve significantly innovated the population management model, in order to better serve both our members and the health plan’s bottom line. Our unique approach combines personalized in-home face-to-face management with convenient digital connectivity and advanced, highly current analytics. The result is a comprehensive outcomes-oriented methodology that powerfully integrates all the relevant factors into a single unified plan for success. As a result, we have demonstrated a cost-reduction between 30-60% within the first year of managing a member, with significant health improvements which have sustained into subsequent years.

Leveraging member (patient) generated data in conjunction with claims and clinical data is unique in this space. When you couple this with predictive modeling to provide guidance as to the actions which are likely to make the biggest improvement in outcomes with experienced, discerning providers to ensure efficacy, it is game changing. These sources of data are integrated to provide a broader, multi-perspective view of a person. The sophisticated analytics allow for focus to make the appropriate impacts for member success.

Explain how you (your team) will execute to make this idea/product successful? What gives you (your team) an advantage over others already in the market or new to this market?

Our highly skilled team of social, behavioral and physical medicine experts leverages our proprietary technology along with our members on a daily basis. We are engaging ours members throughout their health journey by supporting them in achieving their health goals, while also expanding their knowledge and understanding. Members earn points toward additional health and wellbeing resources through a gamification aspect, which increase utilization of the technology even further and support the improvement of individual’s health outcomes. Most organizations in the population health management space utilize technology in a stand-alone model. Given we highly integrate it into our work in a hands-on manner, adoption and daily usage by our members are significantly greater. Additionally, we share the data with the member's permission with their other providers which give them additional feedback to help in their successes and journey toward improving their health, which further perpetuates their utilization and the overall success.
